摘要 目的 本研究通过检测P14ARF基因、HPV-DNA在正常宫颈组织、宫颈上皮内瘤样病变组织(cervical intraepithelial neoplasia, CIN)、宫颈癌组织中的表达,探讨P14ARF基因、人乳头状瘤病毒(Human papillomavirus,HPV)与宫颈癌发生的关系,并分析P14ARF基因、HPV在宫颈癌的发生发展中两者的关系。 方法 用免疫组化S-P法检测正常宫颈组织20例、宫颈上皮内瘤样变组织10例、宫颈癌组织50例中的P14ARF基因的表达,比较不同宫颈组织中P14ARF基因表达的阳性率。用PCR法检测同样标本中HPV-DNA表达,比较不同宫颈组织中HPV-DNA表达的阳性率。 结果 P14ARF基因和HPV-DNA在宫颈癌、宫颈上皮内瘤样变组织中的阳性表达率明显高于正常宫颈组织中的表达,差异有统计学意义(P<0.05)。在宫颈癌组织中P14ARF基因阳性表达率与组织学病理分级、肌层浸润深度有关,差异有统计学意义(P<0.05),其阳性率逐级上升;与临床分期、淋巴结转移无关(P>0.05)。HPV-DNA的阳性表达率与组织学病理分级、肌层浸润深度、临床分期均无关(P>0.05)。HPV的持续感染与P14ARF基因的表达呈正相关(Spearman相关系数γ=0.432,P=0.002<0.05)。 结论 随着正常宫颈→CIN→宫颈癌的演变,P14ARF基因、HPV-DNA的表达逐渐增强,而且在宫颈病变中HPV的持续感染与P14ARF基因表达呈正相关。因此认为P14ARF基因过度表达、HPV的持续感染与宫颈癌发生、发展过程有密切的关系,这还可能与肿瘤浸润、转移及不良预后有关。
外文摘要 Objective To study the expression of P14ARFgene、HPV-DNA in cervical tissues of differential pathological types, especially in cervical intraepithelial neoplasia (CIN) and cervical carcinomas and determine the role of P14ARFgene、HPV in the progression from CIN to invasive cancers. Methods Immunohistochemical staining S-P method was used to detect the expression of P14ARF in 20 cases of normal cervical tissues、10 cases of cervical intraepithelial neoplasia(CIN) and 50 cases of cervical carcinoma.PCR technology was used to detect the expression of HPV-DNA in same cases. Results The positive expression rate of P14ARF、HPV-DNA in cervical carcinoma and cervical intraepithelial neoplasia(CIN) were higher than that in normal cervical tissues(P<0.05). There was association between P14ARF expression and histological grade、muscular invasion(P<0.05), with the increase of histological grade and muscular invasion, the expression of P14ARF was increased, but there was no association between P14ARF and surgical stage、lymphatic metastasis . There was no association between HPV-DNA expression and histological grade、surgical stage and muscular invasion;positive correlation was also found between P14ARF and HPV-DNA expession(Spearman r=0.432,P=0.002<0.05). Conclusion The variation of P14ARF expression and HPV-DNA persistent infection in cervical carcinoma may contribute to the occurrence and development of the cervical carcinoma.
